Process FMEAManagement system

See in advance what can go wrong in a process

FMEA lets you map in advance what can go wrong in a process. For each process step you record failure modes with cause and effect and score severity, occurrence and detection, after which the system calculates the risk priority number (RPN). Additional measures become actions, and the residual risk shows that they work.

What you do with Process FMEA

Studies built on your processes

An FMEA study belongs to a process from your process structure. You break the process into steps, or start from example steps and adapt them.

Worksheet per process step

For each step you record failure modes with potential cause, potential effect, current prevention controls and current detection.

RPN calculated automatically

You score severity (S), occurrence (O) and detection (D) from 1 to 10 each, and the system calculates the RPN from 1 to 1000.

Action required at threshold

When the RPN reaches the threshold (100 by default, adjustable) or severity is 8 or higher, the failure mode is marked as requiring action. High severity cannot be averaged away by low probabilities.

Measures and residual risk

An additional measure goes straight to Actions, with the failure mode as its origin and its live status shown in the worksheet. You then score the residual risk, which from then on counts as the current risk.

AI draft list, PDF worksheet

AI suggests possible failure modes based on the process, scope and steps; you decide per suggestion to add, edit or reject it. The PDF worksheet in your house style is ready for the auditor.

In practice

How it works

  1. 1

    Start a study

Create a new FMEA in four steps: what, scope with owner and reassessment date, process steps and summary.

  2. 2

    Record failure modes

Work through the worksheet step by step and score each failure mode. Once there is at least one failure mode, you move the study from draft to active.

  3. 3

    Assign measures

Where action is required, add an additional measure with owner, deadline and priority.

  4. 4

    Score the residual risk

After the measure you score S, O and D again and see whether the risk drops below the threshold.

  5. 5

    Reassess

When the reassessment date passes, the owner automatically receives a task and a notification.

A selection of what AI does in Process FMEA

In Process FMEA, AI proposes conceivable failure modes and answers questions about your FMEA data; nothing is ever recorded automatically.

Propose failure modes

AI proposes conceivable failure modes based on the process, the scope and the existing process steps.

Cause, effect and starting scores

Each proposal shows a cause, an effect, a rationale and indicative starting scores.

Add, edit or reject

For each proposal you choose to add it, edit and add it, or reject it with a reason.

Questions about your FMEA data

On the AI tab you ask, for example, which failure modes have the highest RPN.

This is a selection of the AI capabilities in this module, not everything. AI suggests, you decide. Nothing is applied without your approval.

Frequently asked questions

Questions about Process FMEA

Why S×O×D and not Kinney & Wiruth?

FMEA is by definition Severity × Occurrence × Detection, and the detection question, would we notice it, is exactly what sets it apart. Kinney & Wiruth remains the method for the occupational risk register of the risk assessment.

Does the initial or the residual RPN count towards the threshold?

The current risk: the initial RPN until a residual risk is scored, the residual RPN after that.

When should I reassess an FMEA?

When the process changes, after incidents or complaints from the process, and no later than the reassessment date. The system tracks that date for you.

Does the AI record failure modes on its own?

No. The AI draft list offers suggestions with cause, effect, rationale and indicative starting scores. You decide per suggestion to add it, edit and add it, or reject it.

Start small

Start with just Process FMEA and expand when you want

You do not have to set up everything at once. Switch this module on, add other modules later and keep using the same data and the same app. BrydgeOne fits every organisation, from a small company to enterprise, and you only pay for what you switch on, from € 49 per month.
